二、申請人的資格要求：

合同包1(Q開關(guān)NdYAG激光治療機采購項目)特定資格要求如下:
(1)（1）提供參與本項目投標供應商有效的醫(yī)療器械經(jīng)營許可證；（2）提供參與本項目投標供應商有效的食品藥品經(jīng)營許可證，經(jīng)營范圍包括三類醫(yī)療器械；（3）提供參與本項目投標供應商有效的醫(yī)療器械生產(chǎn)許可證； （1）—（3）任意提供一項即可。

七、其他補充事宜
一、關(guān)于依法繳納社會保障資金的證明材料的說明：
（1）本項目采購文件中所稱“社保經(jīng)辦機構(gòu)”是指：《社會保險經(jīng)辦條例》（中華人民共和國國務院令第765號）第六十條中“社會保險經(jīng)辦機構(gòu)”，即人力資源社會保障行政部門所屬的經(jīng)辦基本養(yǎng)老保險、工傷保險、失業(yè)保險等社會保險的機構(gòu)和醫(yī)療保障行政部門所屬的經(jīng)辦基本醫(yī)療保險、生育保險等社會保險的機構(gòu)。
（2）提供項目開標前半年內(nèi)任意連續(xù)3個月（不含投標（響應）當月）的《繳納社會保障資金的證明材料清單》或提供依法繳納社會保險資金證明材料。
二、根據(jù)慶財采【2024】17號文件要求，本項目在法定公示內(nèi)容基礎(chǔ)上將依據(jù)參與供應商自愿原則，在中標結(jié)果公告中公開投標（響應）文件中的投標承諾書、特定資格證明文件、中小企業(yè)聲明函/監(jiān)獄企業(yè)/殘疾人福利性單位聲明函、投標人業(yè)績情況表、報價明細表、開標記錄表。如同意請在投標響應文件中提供承諾函（格式自擬，位置不限，內(nèi)容表明“同意”即可），不同意可不提供。
