合同包1(2024年明湖泵站調(diào)節(jié)池清淤項(xiàng)目)落實(shí)政府采購(gòu)政策需滿(mǎn)足的資格要求如下:
本項(xiàng)目專(zhuān)門(mén)面向小微企業(yè)采購(gòu)，大型、中型企業(yè)參與本項(xiàng)目響應(yīng)的無(wú)效。供應(yīng)商自行提供中小企業(yè)聲明函以證明自身企業(yè)性質(zhì)。監(jiān)獄企業(yè)及殘疾人福利單位視同為小微企業(yè)。
3.本項(xiàng)目的特定資格要求：
合同包1(2024年明湖泵站調(diào)節(jié)池清淤項(xiàng)目)特定資格要求如下:
(1)①具備有效的市政公用工程施工總承包三級(jí)（含三級(jí)）以上資質(zhì)（提供原件彩色掃描件）。 ②具有有效的安全生產(chǎn)許可證（提供原件彩色掃描件）。 ③項(xiàng)目經(jīng)理1人，具備有效的二級(jí)（含二級(jí)）以上建造師注冊(cè)證及安全生產(chǎn)考核證（提供原件彩色掃描件），其他人員待中標(biāo)后必須根據(jù)國(guó)家相關(guān)規(guī)定進(jìn)行配備。
(2)具備有效的法定代表人身份證及授權(quán)代表身份證。（提供原件的彩色掃描件） 注：法定代表人參會(huì)時(shí)提供法定代表人身份證即可；法定代表人未參會(huì)時(shí)徐提供法定代表人身份證及授權(quán)代表身份證。
(3)投標(biāo)供應(yīng)商在中華人民共和國(guó)境內(nèi)不得有失信、圍標(biāo)串標(biāo)、弄虛作假等違法行為，供應(yīng)商自行提供承諾書(shū)（承諾書(shū)格式自擬，加蓋公章），否則按廢標(biāo)處理。
